Social	حضرت الأم إلى العيادة تشكو من أن عاليا تعانى من العديد من السلوكيات السلبية المتمثلة فى العصبية المفرطة، التحدث بصوت مرتفع ، الرد على الأم بطريقة غير لائقة،بالإضافة إلى أخذ ممتلكات الغير، سرد الأشياء التى تحدث فى الببيت للغرباء على الرغم من تنبيهها. كما أضافت الأم إلى أنها كثيرة الكلام ، متهورة، ترفض أداء الواجبات المكلفة بها. • معلومات دالة ومنتقاه : - الولادة : قيصرية. - تعانى عاليا من عادة مص الملابس و الجز على الأسنان. - تبول ليلى لاإرادى ثانوى على فترات متباعدة. - تعانى من آحلام مزعجة (كوابيس). - التحقت عاليا بحضانة خاصة عمر 4 شهور واستمرت حتى دخول المرحلة التمهيدية ولم ترد شكاوى سوى من فرط الحركة. - الصف الأول والثانى من المرحلة التمهيدية تمثلت الشكوى من عدم اتباع التعليمات ومن فرط الحركة. - الصف الأول من المرحلة الإبتدائية: شكى المدرسون من التعدى البدنى على الزملاء ومن رفض نقل المادة المتعلمة من على السبورة. - قد يلجأ الأهل للعقاب البدنى كوسيلة للتهذيب. • التقييم النفسى والملاحظات الأكلينيكية: كانت عاليا أثناء جلسات التقييم النفسى متعاونة، تظهر أداءا ايجابيًا فى الأنشطة المعرفية والعقلية الموازية لعمرها الزمنى ، وإن أظهرت بعض مظاهر اضطراب نقص الانتباه وفرط الحركة حيث كانت متململة، تقوم بفرك يديها على الترابيزة، مندفعة الأداء، سريعة الملل ، تعبر عن رغبتها فى انهاء النشاط على الرغم من عدم انهاءه، كثيرة الكلام، وأخيرا كانت تميل للزن والإلحاح فى طلب الأشياء التى تريدها. ولقد تم تطبيق الاختبارات الاتية: 1- كونرز: Conner's Rating Scale يقيس، هذا الاختبار المظاهر الآتية وقد أجاب الأهل على بنود الاختبارالحالى وقد كانت النتائج كما يلى: Guide Lines T/ Score subscale Markedly Atypical (Indicates significant problem) 73 Oppositional (A) Markedly Atypical (Indicates significant problem) 90 Cognitive problem/Inattention(B Markedly Atypical (Indicates significant problem) 90 Hyperactivity (C) Average 45 Anxious-Shy (D) Mild Atypical (possible significant problem) 64 Perfectionism(€) Average 52 Social problem (F) Slightly Atypical (Borderline: should raise concern) 56 Psychosomatic(G) Markedly Atypical (Indicates significant problem) 81 Conner's ADHD Index(H) Markedly Atypical (Indicates significant problem) 83 Conner's Global Index Restless Impulsive(I) Markedly Atypical (Indicates significant problem) 83 Conner's Global Index: emotional labiality(j) Markedly Atypical (Indicates significant problem) 83 Conner's Global Index: Total(k) Markedly Atypical (Indicates significant problem) 90 DSM-IV: Inattentive(L) Markedly Atypical (Indicates significant problem) 90 DSM-IV: Hyperactivity/ Inattentive(M) Markedly Atypical (Indicates significant problem) 90 DSM-IV: Total(N) تشير النتائج إلى وجود ارتفاعات دالة فى مختلف المظاهر التى يقيسها المقياس كما هو موضح بالجدول فيما عدا Social problem ، Anxious-Shyاللذان يقعان فى المستوى المعتدل. 3- المقياس السلوكى للوظائف التنفيذية BRIEF المهارات T/Score الكف/ المنع :Inhibit 68 التحول: Shift 64 التحكم فى الانفعالات: Emotional Control 73 تنظيم السلوك: Behavioral Regulation 71 المبادرة: Initiate 55 الذاكرة الوظيفية: Working Memory 75 التخطيط/ التنظيم: Plan/ Organize 74 تنظيم الأدوات Organization of Materials 52 المراقبة : Monitor 73 ما وراء المعرفة : Meta cognition 70 الوظائف التنفيذية الكلية Global Executive Function 73 (الدرجة الدالة > 65) تشير النتائج السابقة إلى وجود ارتفاع دال فى مظاهر Inhibit ، Emotional Control Working Memory، Behavioral Regulation Monitor، Plan/ Organize، Monitor،Meta cognition، Global Executive Function التى يقيسها المقياس بينما كانت باقى المظاهر فى المستوى المعتدل. 1- قائمة وصف سلوك الأطفال والمراهقين:- أجاب الأهل على هذه القائمة وقد كانت النتائج كما يلى: المشكلات السلوكية والنفسية T/ Score السلوك العدوانىAggressive Behavior 65 السلوك الأنحرافى Rule Breaking Behavior 66 مشكلات الانتباهAttention Problems 80 مشكلات التفكيرThought Problems 73 المشكلات الاجتماعية Social Problems 53 القلق والاكتئاب Anxious/ Depressed 50 المشكلات الجسدية Somatic Complaints 53 العزلة/ الاكتئاب Withdrawn/Depressed 55 (الدرجة الدالة > 70) تشير النتائج السابقة إلى وجود ارتفاع دال فى مشكلات الانتباهAttention Problems ، مشكلات التفكيرThought Problems بينما كان السلوك العدوانىAggressive Behavior ، السلوك الأنحرافى Rule Breaking Behavior فى المستوى البينى فى حين كانت باقى المظاهر فى المستوى المعتدل. وعند الرجوع إلى التوجهات التشخيصية الرئيسية للدليل الامريكى الرابع ( DSMIV) التى تظهر من تطبيق المقياس يتضح وجود ارتفاع على T/ Score فى ,(76) conduct problems ، Attention deficit/hyperactivity problems (76)، بينما كانت(65) oppositional defiant problems فى المستوى البينى ا.
