Summary	شاهندة ٣٣ سنة، بكالوريوس رياض أطفال، تعمل مدرسة لغة انجليزية، متزوجة و لديها بنت ٥ سنوات، تشكو من اعراض قلق وتوتر و اعراض تشبه نوبات الهلع و افكار وسواسية و اعراض اكتئابية متوسطة الشدة نشأت في اسرة غير متكافئة حيث الأب من طبقة الفلاحين ٦٢ سنة وكان يعمل مراقب صحة ويبدو أن شخصيته غير قوية وسهلة التأثير و علاقتها به عادية، أما الأم فهي من طبقة ارستقراطية الى حد ما وكانت تعمل في وزارة الإعلام وتبدو قلقة دائما وناقدة ومثالية وعلاقتها بها قريبة.. المستوى الدراسي للمريضة متوسط، لها تاريخ من نوبات الصرع الكبرى اول مرة في ٢٠٠٨ و استمرت على دواء ديباكين لمدة سنتان ثم اختفت هذه النوبات، خضعت لعملية استئصال الزائدة وأثناء العملية تم إستئصال مبيض و كي للمبيض الثاني.. عملت لمدة سنةفي مدرسة فكرية و تركتها تحت تأثير والدتها لإعتقاد والدتها بأن العمل مع الأطفال المعاقين سوف يسبب لها إكتئاب بالرغم من اهتمامها بهذا المجال،تم من قبل ارغامها ع الخطوبة بأحد أقاربها ولكن لم تكتمل الخطوبة،، تزوجت عام ٢٠١٣ من زميل لها بالعمل بعد إعجاب، الزوج أكبر منها بسنة وحاصل على ليسانس آداب ويعمل في بنك و يعاني من وسواس قهري في النظافة و أخذ علاج لمدة سنتين و تحسن و هو حاليا لا يأخذ علاج و لديه بعض الأعراض على حد وصف المريضة، والعلاقة بينهم غير مستقرة و بوصفها هو لا يستطيع التحكم في انفعالاته لحد كسر اشياء في البيت و رمي الهدوم في الشارع و الضرب والشتيمة.. بالفحص هي ذو مزاج قلق هتاخد بروزاك ٢٠ مجم و كيوتابكس ٢٥ و لما تتحسن بالتسبة للقلق و الإكتئاب قد يحتاجا الى علاج زواجي.

Social	عندي أفكار وحشة على طول، ان في حرامي مثلا دخلالذقة، او ان سائق التاكسي هيخطفني، ، وده بيعملي قلق و توتر رهيب وفيه افكار تانية مزعجة بتجيلي و على طول قلقانة

Follow Up	٢٠-٤-٢٠١٩ القلق أقل نسبيا و لا توجد نوبات أشبه بالهلع وتشكو من النوم الزائد، ، يتم ايقاف الكيوتابكس و استمرار البروزاك- ٢١-٥-٢٠١٩ تبدو قلقة ، توقفت عن أخذ الدواء منذ اسبوعين،، الدوا كان يسبب لها غثيان و فقد الشهية لكنها تخبر انها توقفت الدوا بدون سبب واضح، المهم ان أعراض القلقىموحودة و لكن بصورة أقل، و يوجد أعراض إكتئاب خفيف مثل فقدان الطاقة و احساس بالتقصير و الذنب، تشوه في صورة الذات،، هتاخد سيبرالكس ١٠ مجم نصف قرص و تم تعليمها الفرق بين المشاعر والأفكار و السلوم و القرارات.
