والهلع وبعض سمات وسواسية منذ ١٢ سنة مما اثر على ثقته بنفسه واكتسابه مهارات حياتية، وهو حاليا على بروزاك ٤٠ واربيبركس ١٠، سوف تخفض الى خمسة مع اضافة منيبرس وجروب ثيرابي وعلاج معرفي سلوكي اسئلة وجودية احنا مين احنا فين وفِي فرق بيني وبين الواقع، وعارف ان ده مش مظبوط وفيه خوف من الموت باستمرار، ودايما خايف، وفيه بانيك اتاك وقالولي ديبرسوناليزيشن.، Allergy Family History Family History No Yes Follow Up ٨/٨/٢٠١٩ ذهب لدكتور سالي عدة مرات وتحسن كثيرا ولكن انتكس عدة مرات، ومازال يعاني من القل المستمر والتوتر وفقد الثقة بالنفس وكذلك اعراض انشقاقية مثل تبدد الشخصية وتباعد عن الذات (إحساسي مش منطبق على جسمي، وانا مش انا وان الدنيا مش حقيقة زي ماتكون وهم ) ودايما عندي إرهاق ومنذ شهر رجع لحبتين بروزاك ٤٠ واريبيبركس ١٠ مل وبالفحص يعاني من نوبات هلع كل شهرين واعراض تبدد شخصية مستمرة تتزايد مع التعرض للضغوط مع أعراض حزن وقلق وخمول واندفاعية وسوف نغير الاريبيبريكس الى ابيكسيدون ونزود تيبوفورتين وبوسبار ويعمل شغل معرفي سلوكي ثم في مرحلة اخرى يدخل الجروب مسار 25 جلسة 6/7/2020 محمود سعد تم تحديد مجموعة من الأهداف وهى بالترتيب الهلع والقلق واغراض استغراب الذات والاخرين فى الجلسات الاولى تم التغلب بشكل كبير على نوبات الهلع رغم قوتها الشديدة والانتقال إلى أعراض القلق والتى حدث بها تحسن ملحوظ وتغيرت ملامح وجهه للافضل ثم التعامل مع استغراب الذات واظهر تحسن وصل إلى ٦٠% ولكن يبدو احيانا لديه تقلب فى أعراض استغراب الذات تم التعامل مع الأعراض بطريقة التعرض والتعود من خلال التعرض لتمارين الدوخة والتى تزيد من أعراض استغراب الذات وتم التركيز على عدد من الاحساسات منها مراقبة الذات من الخارج والاحساس بكونه مغترب عن نفسه مع التساؤلات الوجودية ويتبقى من الخطة العلاجية المزيد من التحسن فى أعراض القلق واستغراب الذات خصوصا أن تحسن استغراب الذات متوقف عند نقطة واحدة ولا يتحرك، لذا تم التعامل بطريقة التحليل النفسى تعدد الذوات منذ جلستين للتعامل مع استغراب الذات والتحرك من النقطة السابقة ملحوظة تم تحديد أكثر من موعد مع نبيل ولكن المريض كان يعتذر بشكل غير مبرر كذلك تم الإستغناء عن المريض فى العمل وحاليا يعمل اعمال بسيطة من المنزل وسوف يقوم بعمل مقابلة للعمل فى مجموعة صيدليات ٧/٧/٢٠٢٠ فيه تحسن في كل الاعراض ماعدا اغتراب الواقع واغتراب الشخصية وشرحت له ازاي يعمل وعي اني مع الاحاسيس لكي يكون في الواقع اكثر، وحاليا على ابيكسيدون واحد مل، وبروزاك كبسولة يوميا رغم عدم انتظامه فيه مطلوب يكمل مع محمود في الوعي الآني عن طريق الاحاسيس ويمشي على بروزاك يوم بعد يوم لانه ربما بيقلل الاحاسيس ونغير الابيكسيدون الى اريبيبريكس خمسة مل د. رحاب 22/6/2022 خد الفليوزاك بدل البروزاك تعب جدا ً وجاله
