انطباع اولي انه جاي ، يعاني من القلق ومتحمس جدا للكلام، من اسرة كبيرة سوهاجية ثمان اخوة وأخوات وأب متوفي وعمر المريض عشر سنوات ويعاني منذ كان سنه ست عشرة سنة بدا بالخوف على الام من الموت، مع تفكير زايد، ، ، اقرب للاضطراب الوسواسي، وحاول العلاج عدة مرات وفشل، ، Diagnosis/Notes Social بدأ التعب في ثانية ثانوي اني بفكر على طول ورحت لدكتور قالي وراثي، والداني دوا كتير ومبيسمعنيش، وبقيت عايز اقطع شرايين أيدي، ورحت لدكتور تاني ويديني دوا اضحك على طول وبيقولي تحمد ربنا ماعندكش مرض خبيث وقالي عندك هلع، وأعطاني علاج جدول وبطلت اخاف، Allergy Family History Family History No Yes Follow Up ٢٨/٢/٢٠١٨ مازال زهقان جدا، ومتوتر دايما،، ويشتكي من التفكير اثناء النوم، وكثير الشكوى بشكل عام ٢٢/٣ اتصال تليفوني وزود السيروكويل ل ٧٥ مل ١٥/٩/٢٠١٩ مازال في الكلية ويقوم بإدارة عربة نقل لصالح الاسرة، ومتوقف عن الدوا منذ سبع شهور وماشي يعد كل حاجة، وعلى طول في اكتياب ومفيش نوم وتفكير مفرط (اتضح فعلا انه باسيكشوال ولكن محافظ جدا وبدون اَي ممارسة،كما توقعت في البداية وشرحت له الموضوع،،) ويعاني من افكار بارانوية وشعور بالذنب وبعيد كل شيء واي شيء بيتقابلو ولا يتوقف عن العد وكان فيه افكار انتحارية وراحت، وقد اخذ دواء بروزاك وبوسبار وريسبيريدال، وسوف ياخذ انافرانيل ٧٥ ويزداد تدريجيا ٣٠/١١/٢٠١٩ النوم اتحسن والأفكار الانتحارية قلت ولكن الوسواس كما هو ومنذ شهر بدا لا ينام، ويشتكي بشدة من الاكتئاب مثل التفكير السلبي، ومابيتبسطش الا نادرا، والتفكير المفرط وبيشتكي من التأخير في العادة السرية طبعا بسبب الأنافرانيل، وسوف نضيف ابيكسيدون واحد مل لتهدئة التفكير الوسواسي ،وافكار الإشارة ويذهب لمحمود سعد ٩/٢/٢٠٢٠ دهب لمحمود مرة واحدة ولَم يكمل بسبب الامتحانات، وحاليا وسواس الموت بشكل قوي والمصيبة انه من عشر ايّام فقد اخوه الاكبر بسنتين والأقرب اليه ،في حادثة سيارة، ولا ينام، وفِي حالة حداد مؤلم ورغبة شديدة في الموت ولابس اسود ١٧/٦/٢٠٢٠ مازال تعبان جدا، ،والمصيبة انه ابتدى يهيج على اخوه بشكل فظيع وعايز يقوله أديني بتاعك اجض فيه وطبعا زعقتله وغالبا هيحتاج جلسات كهربا وسوف أضيف الأنافرانيل افيكسور ولو منفعش ياخد كهربا ٢/٩/٢٠٢٠ اتحسن في الجنس فقط ٤/١٠/٢٠٢٠ اخذ ثلاث جلسات ورغم تحسنه الواضح يرفض الإكمال لان ضهره ورقبته وجعته وحتى وقف الدوا وسوف يعود الى الانفرانيل ١٥٠ ويكمل مع إسماء ٢٩/٨/٢٠٢١ مازال في الجيش وموقف الأدوية تماما منذ خمس شهور، ويشتكي حاليا من التوتر وخصوصا مع الاخرين مع مشاعر جنسية مثلية، والنوم وحش وسوف ياخذ اميتربتين ٥٠ ٥/١/٢٠٢٢ خلص الجيش من شهر وماشي الى اميتربتين ٥٠ مل، وشكله احسن ولا توجد افكار انتحار ولا توجد وساوس جنسية ولكن مازال في حالة انكار لموت الاخ مزاجه اكتئابي بشكل خفيف مع نوم ضعيف وسوف نزيد الاميتربتين الى ١٢٥ مع كويتيابين ١٠٠ ١٤/١١/٢٠٢٢ استقرت الحالة على امتئاب مزمن مع حداد مرضي مستمر ورافض موت الاخ ويعيش في فلاشباكس مستمرة عليه وبيزق نفسه في الحياة ولكن بلا افكار انتحارية وسوف ياخذ كيتامين في حالة اشتغل في الحكومة ويكمل على الدوا مع زيادة جرعة التريبيزول الى ٢٠٠ مع لاميكتال ١٢/١٢/٢٠٢٢ الوضع سيء وسوف ياخذ تريبتيزول ١٢٥ مع ولنتا والتروكسين ٢٩/٥/٢٠٢٣ اتحسن على تركيبة الادوية السابقة وتوقف عن التفكير في الانتحار ولكن مازال عايش في حالة انكار وفاة الاخ ولا يستمتع بشيء ويشتكي كذلك من نقص الرغبة الجنسية رغم تحسن توجهه الجنسي الغيري وسوف يستمر على الولنتا اميتربتين ١٠٠ والتروكسين ٥٠ ٥/٩/٢٠٢٣ لاول مرة يقول انه اتحسن والحمد لله، ويعيش ويعمل في العالمين ويدير محل موبيلات من الساعة اربعة عصرا الى اربعة صباحا، ولكن يتفي اي استمتاع وينفي الرغبة الجنسية. ام انه يمارس العادة السرية وسوف نزيد الثيروكسين الى ١٠٠مل ٢٤/٢/٢٠٢٤ عموما احسن ويفكر في الزواج ويشتكي سرعة القلق والحزن وانه حتى الان لم يعترف بموت الاخ لانه خايف يموت لو اعترف بذلك وسوف يبدا يدلع نفسه مع بنت ويزيد التريبتيزول الى ١٥٠ ٣٠/٩/٢٠٢٤ رغم التحسن الكبير واستمراره في العمل يعاني بشدة من دمة موت الاخ التي تمنعه عمل اي حاجة مبهجة مثل الزواج او الخروج او الفرح ولكن علاقته
