Social	20 / 12 /2016 السيد/ محمد ع 28، هندسة، أستقالة، أعزب، يعيش مع الأم والأخت الوسطى متزوجة، حلوان. شكوى المريض: رؤيتى أتغيرت، بقيت منعزل، الـ Gestures بتضايقنى، وممكن أفهمها ضدى وممكن لأ. شكوى المبلغ: (الأم والأخت) بقى مختلف، كان اجتماعى، بقى حساس، عنده إحساس بالمؤامرة وشاكك إن حد قتل بابا، وبيربط حاجات ماتتربطش وبقى فيه تهيؤات، واستقال من الشغل. التعقيب: - بدأت الحالة منذ اكثر سنة وزادت بعد وفاة الأب وبدأ يعانى من Hallucination، Paranoid reaction، وان كان تحسن تدريجيا ً ولديه بصيرة بما يحدث. R\ Apexidon 2 mg 1x1. 10/1/17 الدوا عملي صداع، وجابلي كوابيس، هو احسن كتير ويرجع هذا للإقناع، هو اهدا، وال gestures اهدى، والام شكلها مش متحمسة للدواء، تم تغيير الدوا الى اولابكس ١٠ نص، ١١/٧ حصل لخبطة في الدوا وحاليا على ابيكسيدون ٢ مل، احتمال فصام وجداني، لانه اتحسن كتير ولكن فيه أعراض اكتئابية وفيه تقلبات مزاجية محتاجين تظبيط الأعراض ثم ادخاله على التاهيل النفسي، ابيكسيدون ٣ مل ومحتاج اشوفه بعد إسبوعين وادخله الجروب ٢٦/٧ مازال نشط ذهانيا، وحضر الجروب وعمل مشاكل كتير، سياخذ خمسة مل ١٦/٨ احسن، وأهدى، واضح ان الحالة بدات وعمره ٢٥ سنة، وشرحت له كيفية ظهور المرض وكيف ان المخ يمرض مثل باقي الأعضاء ، حاليا على ريسبيريدال خمسة مل، ورغم الأعراض بيدور على شغل جامد ١٣/٩ EPS, بمراجعة الأعراض واضح ان الحالة بدات منذ سنتين ونص منذ وفاة الاب ( يعني فصام حسب التشخيصات ؟!!)، ريسبيريدال ٤ مل ١١/١٠ فيه EBS ونوم كتير، وخوف من العجز الجنسي، وفيه تشاؤم وخوف،

Follow Up	الأخت والام لوحدهما : وقف الدوا منذ ست شهور ورافض الدوا تماما يجيلي وبيخاف تماما ومنعزل ولا يخرج من البيت ١٠/١٠ بعد ثلاث أسابيع مطلوب يدخل المستشفى ١٥/٤/٢٠١٩ بعد الخروج من المستشفى من شهرين ، أتقدم لشغل ومتوتر جدا بسبب ٢٥/٨/٢٠٢١ جاء بعد سنتين ونصف واتحسن جدا وحاليا على لاميكتال ٢٠٠ وابيكسيدون ٢ مل مساء، وحاليا متوتر فقط عندما يتقدم الى عمل مع سرعة القلب والانفعال (القمص) وانخفاض الثقة في النفس وسوف يبدأ عمل قريبا واتفقنا يمشي على الدوا خمس سنين بدون انتكاسة م نفكر في إيقاف الدوا،
