Summary	ياسين طفل يبلغ من العمر 5 سنوات الطفل الاول من اثنين يوجد بين الوالدين مشاكل بسيطة تحدث امامة ، يظهر مظاهر فرط حركة وندفاعية وسلوكيات سلبية شديدة .
Diagnosis/Notes
Social	حضر الاهل يذكرون شكوى المدرسة من أن ياسين لا يتبع التعليمات الموجه اليه، الميل لتقليد السلوكيات السلبية للاخرين، كثرة الالحاح والزن عند الرغبة فى شئ ما، بالإضافة إلى أنه مفرط الحركة، مندفع الاداء، اللعب بعنف مع الزملاء والتعدى البدنى عليهم. وقد ذكرت الام ان ياسين لا يظهر اى من هذه السلوكيات داخل المنزل، وان كان يقطع الكلام ، بين الابوين، مفرط الحركة، مش بيسمع الكلام خالص لونزلنا ورحنا عند حد . توجد خلافات بين الام واهل الاب . . • معلومات دالة ومنتقاه تم اخذها من الأهل : - الولادة: قيصرية. - لا يوجد تأخر فى مختلف مظاهر النمو. - بدء هذا العام يعانى من عادة قضم الاظافر ومن وضع يده فى فمه. - التحق آدم بعدة حضانات خاصة عمر عامين و7 شهور ولم ترد شكاوى منه سوى انه مفرط الحركة وانه كثير البكاء اذا رفض اصحابة اللعب معه. - الصف الاول من التمهيدية تكررت الشكاوى السابقة - يلجأ الأهل إلى اتباع اسلوب العزل والحرمان من الأشياء المحببة كوسيلة للتهذيب. • التقييم النفسى والملاحظات الأكلينيكية: كان ياسين اثناء التقييم النفسى متعاونًا ، يظهر اداءا ايجابيًا فى بعض الانشطة الموازية لعمره الزمنى وان كان يظهر ضيقًا من الانشطة التى تحتاج لمجهود زهنى ، كما مظاهر داله لإضطراب نقص الانتباه وفرط الحركة حيث كان مُفرط الحركة، يرفض الجلوس لأداء آى مهمة تتطلب منه، يعبث بالأدوات المحيطة، ، مندفع الأداء، مشتت الانتباه ، لا يتبع التعليمات الا مع تكرارها ، كما كان يضع الملابس فى فمه، وعندة سؤال ياسين عن سبب قيامة بضرب زملائه اشار انهم بيضايقونى ولقد تم تطبيق الاختباركونرز: Conner's Rating Scale يقيس هذا الاختبار المظاهر الآتية وقد أجاب الأهل على بنود الاختبارالحالى وقد كانت النتائج كما يلى: Guide Lines T/ Score subscale Mildly Atypical (Possible significant problem) 61 Oppositional (A) Moderately Atypical (Indicates a significant problem) 69 Cognitive problem/Inattention Moderately Atypical (Indicates a significant problem) 68 Hyperactivity (C) Markedly Atypical(Indicates significant problem) 79 Anxious-Shy (D) Moderately Atypical (Indicates a significant problem) 67 Perfectionism(€) Slightly Atypical (Borderline: should raise concern) 52 Social problem (F) Markedly Atypical(Indicates significant problem) 87 Psychosomatic(G) Markedly Atypical(Indicates significant problem) 77 Conner's ADHD Index(H) Markedly Atypical(Indicates significant problem) 72 Conner's Global Index Restless Impulsive(I) Mildly Atypical (Possible significant problem) 62 Conner's Global Index: emotional labiality(j) Moderately Atypical (Indicates a significant problem) 70 Conner's Global Index: Total(k) Slightly Atypical (Borderline: should raise concern) 56 DSM-IV: Inattentive(L) Markedly Atypical(Indicates significant problem) 75 DSM-IV: Hyperactivity/ Inattentive(M) Moderately Atypical (Indicates a significant problem) 69 DSM-IV: Total(N) تشير النتائج إلى وجود ارتفاعات متباينة فى العديد من المظاهر التى يقيسها المقياس كما هو موضح بالجدول. 2- كونرز صورة المدرسة : Conner's Rating Scale يقيس هذا المقياس المظاهر التالية . الابعاد المدرسين Conner's ADHD Index Hyperactivity Cognitive problem/Inattention Oppositional لغة عربية 76 90 52 70 لغة انجليزية 79 90 44 47 رياضيات 75 90 52 53 تشير النتائج السابقة إلى اتفاق مختلف المدرسين إلى وجود مظاهر دالة لفرط الحركة ، بينما كان هناك ارتفاع للسلوكيات السلبية لدى مدرس اللغة العربية بينما كانت باقى المظاهر فى المستوى المعتدل والطفيف من حيث تواجد الاعراض. 3- مقياس انتباه الأطفال و توافقهم صورة المدرسة ( CAAS ) : الأبعاد الدرجة المعيارية اللغة العربية الدرجة المعيارية اللغة الانجليزية الدرجة المعيارية الرياضيات نقص الانتباه 100 113 104 الإندفاعية 101 95 95 تشت الانتباه 101 108 101 فرط النشاط 116 116 116 نقص الانتباه وفرط النشاط 115 119 115 السلوكيات السلبية 116 109 117 ( متوسط الفرد العادى: 100 + 15 ) تشير النتائج السابقة إلى وجود اتفاق بين مختلف المدرسين ان ياسين يعانى من مظاهر دالة لفرط الحركة وبعض السلوكيات السلبية لدى مدرس الرياضيات واللغة العربية
