مهندس في شركة مقاولات يسكن في بني سويف متزوج من ٢٠١٣ ولديه ولد وبنت لاب محامي والام مدرسه عمل في السعوديه بعد التخرج وعاد في ٢٠١٦ بعدها استمر ٦ شهور رافض يعمل بعدها يعمل في مكتب في المنيا بمرتب قليل جدا وهو راضي وخلاص لغاية ماهم مشوه وأبو الزوجه جابله شغل و ساب الشغل بعد شهر وسافر السعوديه ورجع بعد سنه من السعوديه وهناك ماشتغلش وابوه يصرف ورجع مصر واشتغل وشويه وساب الشغل ( بدا يقول كلام انه يضرب مديره وأنه مضطهده وعاوز يأذيه ، وشتم ابوه و امه) بيشتكي من أعراض شبه ال اضطراب وجداني ثنائي القطب سيتم عمل اختبارات نفسيه مع ا علاء وتم وصف ديباكين ٥٠٠ ملج قرصين كيوتابكس ٢٠٠ اريبيبرازول ١٥ وتم تحديد معاد مع د نبيل لتسلم الاختبار و التقييم لطلبهم ذلك Diagnosis/Notes Social Allergy Family History Family History No Yes Follow Up ١٦/١ بدات الحالة الحالية منذ ٣ سنين بعد نزوله من السعودية وذلك بشعور خوف وعزلة في البيت لمدة ست شهور ثم عمل بشكل متقطع ومنذ سنتين في بداية الصيف أصيب بنوبة هوس مع حركة شديدة وقلة نوم وهجوم على الاب والام وجلوس طويل على المقاهي، وبعدها أصيب باكتئاب مرة اخرى ومنذ سنة أصيب بنوبة هوس ثانية وجاء الان يعاني من اكتياب وبفحص التاريخ العائلي الاب محامي يبدو نتن ومعفن لدرجة ان أهل البنت هم اللي بتجيب الاكل والشرب من اَهلها وهو بيشتكي ان البنت مأثرة عليه بسبب نومها الكثير؟؟؟ مع ملاحظة انهم عايشين في الاب، وفِي النهاية هو يعاني من اضطراب وجداني منذ ثلاث سنوات وتتكرر النوبة سنويا والنوبة حاليا ميكس ويكمل على الديباكين والكويتابكس وزودنا فلوزاك ٦/١ الوضع كما هو عليه، راقد في السرير طول الوقت ولا يتكلم في شيء وخرج فقط مع الاب بالعافية ورجع احسن شوية وهو يبدو مكتئب جدا، وزودنا الفلوزاك الى ثلاث حبات وبرافاماكس ٢٠/٣ ٢ فلوزاك ٣ افيكسور ولاميكتال ٢٠٠وبرافاماكس ٢ ٢٧/٤/٢٠١٩ فاق تماما واتغير خالص وابتدى يدور على شغل وعمل اخلاء طرف، ومستمر على بروزاك ٤٠، وافيكسور ٣٠٠، وبرافاماكس ٤٠٠ مل ولاميكتال ٢٠٠ ومشكلة تاخر القذف اتحلت، وبالمراجعة اتضح انه سكيزويد وعنده نقص في مهارات الحياة، وكذلك في الثقة في النفس وصورة النفس ومحتاج شغل معرفي محدود في هذا الامر تعمل عمل 3 جلسات خلالها تم تقييم الاكتئاب وكان 30 فى فئة الاكتئاب البسيط ، بعدها تم عمل تعليم نفسى ما وراؤ معرفى عن الاعراض، وحث المريض على. البحث عن عمل ومحاولة تعليمة ادارة الصراعات داخل الاسرة، وتدريب المريض على رصد الافكار والاجترارات السلبية والتدريب على اياف الاجترارا من خلال تكنيك امتلاء العقل المنفصل، واسلوب التدريب على الانتباه الخارجى، المريض افضل نسبيا حتى الان فى المزاج ٢/٧/٢٠١٩ انتكس وتحول للهوس وموقف الدوا، والاب والام اتخانقوا معاه وتربط الزوجة بين الخناقة والانتكاسة ١٢/٢/٢٠٢٠ دخل ابو العزايم قعد شهر وخرج ومتابع معاهم حتى الان، وماشي على كوبيتاكوول ٣٠٠ وزسبرين ٢ وديكادول ٥٠٠، وداخل في نوبة اكتياب وقفنا الزسبرين وتزودت الديكادول الى ١٥٠٠ وقللت الكويتاكوول الى ١٠٠ وأضفت ليثيوم ١٩/٥/٢٠٢٠ شكله احسن وبيخرج احيانا ولكن المشكلة في عدم وجود شغل ومن ناحية اخرى الزوجة قافشة عليه جامد لدرجة بتقوله انا في حكم المطلقة او حوزي ميت ومقاطعه اهله حتى في رمضان ،وحاليا على ديكادول ١٠٠٠وليثيوم ٢٨٠٠، وكويتاكوول ٥٠ ويستمر كما هو ٩/٩/٢٠٢٠ رغم استقرار الحالة النفسية مازال خامل وشاكك في قدراته النفسية ومتوقع الفشل ويربط ذلك بممارسة العادة السرية كل عشر ايّام وجاء معتقدا انه مدمن عادة سرية ، وطبعا تم تفنيد ذلك وبمراجعة الحالة هو غالبا كان ماشي كويس قم حدث تغيير بعد تعرضه لمشاكل ضخمة مالية واتهام بالسرقة اثناء عمله في السعودية ومنع من النزول لفترة وتحوله بعد ذلك واصبح شخص خايف وتجنبي ثم أصيب بالمرض النفسي بعدها، وغالبا هيحتاج الى اعادة تنظيم وتنشيط حياتي ٢٨/٢/٢٠٢١ حاليا على ديكاديل ١٥٠٠، وبراينيل ١٢٠٠مل، وتيبوفورتين ٤٠ مل، وكويتابكس ٢٥مل، وحاليا زي الفل ومنتظم في الشغل في سوهاج ومستقر نفسيا، ولكن بعيد عن عيلته بالشهور والامور زي الفل ٢٠/٥/٢٠٢١ الزوجة قلقانة شوية عليه من بعض السلوكيات الخفيفة ،ومكالمة احمد انه حاسس بلامبالاة او هدوء ٤/٨/٢٠٢١ مشي على كويتكوول ١٠٠ وظبط، وحاليا على بريانيل ودكاديل والحياة زي الفل ولكن تخن جدا غالبا من الاكل المفرط والسكر عالي ، ٢٩/٧/٢٠٢٤ كان عندي منذ سنة ولم يسجل وكان ماشي على بريانيل ١٢٠٠وديباكين ١٥٠٠ والامور مستقرة لدرجة لم يحدث
