ولدين ٣٠، ٢٦ سنة وتتكلم بارتباك وبتعميم وجاءت تشتكي ان الابن الاصغر قد تخرج من الجامعة في كندا وعاد الى مصر منذ سنة وقد عانى من صدمة موت الاب منذ عسر سنوات ولكنه استمر في حياته ومنذ عودته لمصر وهو منعزل تماما في البيت وليس له اَي أصدقاء ومكتئب اغلب الوقت ويفكر في الانتحار، ويبدو انه جاد في الموضوع، وسوف أعطيه الاختبارات واكلمك في التليفون.... كلمته في التليفون وجاء الى العيادة وبالفحص هو يعاني من الاكتئاب منذ عشر سنوات بعد وفاة الوالد وتدهورت الحالة كثيرا منذ سنة بعد عودته الى مصر وسوف يأخذ بروزاك لمدة شهر ثم علاج فردي وجمعي Diagnosis/Notes Social الام لوحدها بدون المريض: بعد وفاة والده من ١٠ سنين اتصدم جامد وعلى طول متضايق ومش سعيد ومش عايز يخرج Allergy Family History Family History No Yes Follow Up ٢/٦/٠١٩ ابتدى البروزاك من شهر، وفيه شوية لخبطة في النوم ولكن مزاجه احسن، كتير واتضح ن الاكتئاب بدا منذ ١٠ سنوات مع وفاة الاب ومع الجامعة والبعد عن الدعم الرئيسي ومن ثلاث سنوات الحالة تدهورت اكثر وبالذات مع التوقف عن العمل والعودة الى مصر ومع الدواء اتحسن جدا جدا وحاليا محتاج شوية تدخل فردي ويركز على العلاقة والدعم وكذلك الجروب، مع ملاحظة انه هيسافر بعد شهر ونص لكندا وبالتالي محتاجي السرعة ١٥-٦-٢٠١٩ قعدت معاه، هو شاب لطيف و هادي، مزاجه أفضل و بقى يلعب رياضة سباحة و جيم و جري، و بيزور أقاربه، كان في حالة اكتئاب شديدة و مزمنة، هو عاش فترة الإبتدائية في السعودية لظروف عمل الأب كمحاسب في السعودية ، بعدها هاجر لكندا هو و أخوه الكبير و والدته و مكث الأب بالسعودية للعمل، فترة الإعدادية و الثانوية بكندا و ضغط التأقلم و الإندماج مع المجتمع الكندي وصعوبال اللغة والتواصل عند الأم، ثم وفاة الأب ( الأب كان يعاني من سمنة مفرطة و عمل عملية تدبيس معدة و توفي بعد العملية، لم يستفاق من البنح ) في نهاية المرحلة الثانوية و ضغط المذاكرة ثم عودة الأم لمصر لصعوبة تأقلمها مع المجتمع الكندي و مكوث المريض بمفرده و بداية المرحلة الجامعية، كل هذه الضغوطات أدت الى اكتئاب شديد و مع ذلك نجح المريض في تخطي السنة الأولى و الثانية والثالثة من كلية العلوم و مع شدة الإكتئاب قم بتأجيل سنة و العودة لمصر لمحاولة لطلب الدعم من الأسرة والعائلة و يبدو ان هذا ادى الى زيادة لوم النفس و الإحساس بالتقصير و الذنب مع مقارنته بأخيه الذي نجح في تخطي هذا، علاقته بأخيه مش قريبة و مش بيشوفوا بعض غبر مرة كل سنة تقريبا،،هشوفه مرة أسبوعيا لحد ما يسافر ونشوف ٢٢-٦-٢٠١٩ التحسن كما هو وهو ذكي و لاقط الأفكار والحالة اللي كانت موجودة و تأثيرها على علاقاته بأخوه ووالدته وبالمناسبة هيسافر هو ومامته و هيزوروا أخوه هناك لمظة شهر مع انه مكانش بيكلم اخوه هناك طول الفترة اللي فاتت و ده هيكون دعم كويس ليه لما يرجع يستقر هناك، اتكلمنا عن خططف في الشغل لما يسافرو انه عايز يدرس حاجة تانية، ، هو عنده قلق من الإنتكاسة فهمتوا اننا محتاحين نتحرك في مختلف المجالات زي الشغل، الدراسة العلاقات الإجتماعية، الرياضة و اهمية الإنتظام على الدواء الفترة دي، و هيقابل دكتور نبيل قبل ننا يسافر. ٦-٧-٢٠١٩ اشتغلنا شوية معرفي و دعم و تطمين و وضع خطة مستقبلة،، هو مسافر الأسبوع الجاي و هيقابل دكتور نبيل بكرة، و محتاج تقرير لحالته عشان ياخد الدوا معاه و هو مسافر او لما يروح هناك يصرف الدواء بصورة أسهل ٧/٧/٢٠١٩ تقريبا خف واتفقت معاه يكمل كاونسلينج اونلاين مع احمد صلاح وسوف يستمر على الدوا لمدة سنتين ٥/٦/٢٠٢٠ مكالمة تليفون مع الام: وليد في كندا وماشي كويس، واشتغل ونفسيا كويس، واتفقت معاها نعمل جلسة معه ثم حكت عن ،الاخ الاكبر حازم الذي يغير من الاخ وليد رغم وعوده للام برعايته قام بطرده من بيته وطلب منه ترك المدينة التي يعيش فيها وهي غير قادرة على التعامل مع الابن ٢٩/١/٢٠٢٢ وليد: ماشي منذ ثلاث سنين الى البروزاك، وانتقل لكندا منذ سنتين ونص واشتغل والامور ماشية وذهب للعيش في نفس مدينة الاخ ولكن انصدم من معاملة الاخ له ومع ذلك ينظم حياته وبدا يعمل اجتماعيات ويلعب تايكوندو ورجع علاقاته وحتى بدا يقابل بنات ثم جاء كوفيد وانقطعت الحياة تماما واكتأب الناس حوله وزادت الانتحارات حتى ان احد اصدقاؤه انتحر قريبا، وبدا يكتئب
