Summary	19 / 6 /2016 السيد/ ايهاب ش 41، دبلوم تجارة، لايعمل أو يعمل أعمال مختلفة، أعزب، يعيش مع الأم، إمبابة. شكوى المريض: كنت باسمع أصوات من 2003 ومش قادر أسيطر على أفكارى ورحت مستشفى المطار وبعدين رحت لهشام حتاتة وكنت بازود فى الكوجيتول، مبطل مخدرات من 2009. شكوى المبلغ: (الأخت) منفرده كان بيشرب بشراسه مكيفات، بعد ما ولدى توفى أبتدا يجيله واوس وأوهام (فلانه بتحبنى)، وبدأ يتعالج، وكان بيتعالج عند د. هشام حتاته، وأتخانق مع حد ودخل الحبس شهرين تلاتة بتهمة شروع في سرقة، ومؤخرا ً باع محتويات الشقة وودناه مصحة وحاليا ً هناك. التعقيب: - Psychosis (Drug induced- Schizophrenia) R\ Apexidon 3 mg- Depakin 500 mg. ٢٤/٧ الفرق كبير بين قبل الدوا وبعد الدوا حاليا افضل كتير احتمال ، هو؛ لخبطة جامده عن السحر وكيف اثر عليه، ٢٧/٨ المعالج؛ هو متحسن وبيعمل اللي عليه هو؛ العلاج كويس بس زيادة عليا، يشتكي من المشاكل مع الزملا، اكثر تماسكا استمرار كلامه عن السحر، وحكى انه بيقرف من المهبل منذ سبع سنوات بسبب علاقة وتفحص فيها المهبل، (واضح انه بيتحسن أسرع من توقعي) Apixidon 4ml day depakin 500 ٣/١٢ احسن كتير، ورعي كتير وكلام روحانيات وزعلان من أخواته عشان دخلوه المصحة، الأخ والام ، فرط الحركة اقل، وتفكير كتير، اختبارات: ثقة، لوم نفس، paranoid غضب، ، pseudophilosophical في الغالب indifferentiated psychosis or schyzotypal ابيكسيدون ٤ مل ١٢/٣ الأخ: حاليا قاعد في السرير على طول ومن النادر انه يضحك هو: اخويا هو كمان مريض، وبدات اركز في ديني ( حالق شنبه)، ورياضتي والجري بتاعي، قابلت واحد في ٢٠٠٣ بيعرف طبائع الناس وقريت كتاب قديم ولما استخدمت منه حاجة سمعت أصوات، وابتديت أتعالج من وقتها، حاليا على ابيكسيدون فقط ومحتاج مراجعة الحالة مع الام والأخت للتأكد من اختفاء الأعراض والشغل على علاج الواقع اكثر، , ، ١/٧ ملتحي وداخل في دور تدين، وعايز يوقف العلاج، واجهته بتوقفه ووافق انه واخد قرار بالتوقف عن الحياة، لانه عاجز عن الحياة، ومطلوب البحث عن العمل، وعايز يبطل سجاير، Apexidone 4m, anarchol 1/2، 19/10 لقيت صعوبة في التعرف عليه وكما هو، عصبي، وراح اشتغل في توزيع ميه لمدة شهر وسابها، ووقف الاناركول لانه تعبه، ومتعصب وزعلان اني بقاطعه، ومتنرفز جدا من اللي بيطلب منه يشتغل، وبيروح لشيخ تجريد لكن مضايق منه عشان بيقوله اشتغل، واضح schizophrenic وعايز يوقف الدوا عشان أهله شايفينه مريض بسبب الدوا، وطلبت منه مباشرة ياخد الدوا قدامهم او من وراهم، اضافة تريبتيزول مع المتابعة مع علاء ٣٠/١٢ الام والاخ فقط، هو مش منتظم ع الدوا، وعملنا مشاكل، ومش عايز يعمل حاجة ولا يشتغل ولا يمد أيده في حاجة، ولا مديناش فلوس يبيع الحاجة اجهزة كهربية وعفش وكل شوية يخرف مرة مسيحين ومرة ألَّبت اللي قدامنا، وهم ناس غلابة وعلى قدهم ومش قادرين يكملوا، مطلوب إقناعه بالانضمام لمجموعة التاهيل النفسي او مجموعة وسط البلد، مفاتيحه الكلام في الثقافة والتاريخ والسياسة ٣١/١٢ هو لوحده، متماسك كثيرا، وافق بعد مجهود انه متوقف بقى له خمستاشر سنة وان حياته واقفة، ولكن مش قادر أواجهه بمرضه وعرضت عليه يحضر مجموعة علاجية ورفض، وخايف من تكرار الجروبات والمواجهة ١٧/٣/٢١٨ اكثر تماسكا لكن رغاي جدا جدا، ومستفز من أسئلتي عن الشغل والعلاقات، لاني سألتها قبل كده، وابتدى يفكر بشكل واقعي مثل تكملة الدراسة والتفكير في شغل،

Follow Up	٢٩/٦/٢٠١٩ ماشي على فلونيكسول اسبوعين وابيكسيدون ٣مل، والدنيا مظبوطة ومستقرة تماما ولكن الدوا مآثر جنسيا وعايز يوقف الدوا، وسوف يمشي على فلونيكسول ٢ والحقنة، كل ٢٠ سنة ٢٦/١/٢٠٢٠ مستقر تماما، واشتغل امن ولكن ساب الشغل بسبب لخبطة مواضيع النوم وفيه صعوبة في الحصول الى فلونيكسول وكتبت له كلوبيكسول ومستمر على الابيكسيدون ٢، ٢٥/٨/٢٠٢٠ ماشي زي الفل بينام تسع ساعات يوميا، وبيروح مسرح ومبسوط ومروق، وبيقرا، ومبيشتغلش ولكن يشتكي من ثقل الحقنة عليه ولكن يعاني من وصم الناس له بسبب تاريخه المرضي والادماني رغم تغيره الكبير ويرغب في الانتقال من المنطقة ولكن الام رافضة وحاليا على ابيكسيدون ٣ وفلونيكسول كل شهر ٢٥/١١/٢٠٢٠ حصل مشاكل جامدة وخناقات بسبب ميراث وراح القسم عدة ساعات وخرج ومازال زي الفل، وماشي على ابيكسيدون ٣مل، ونص الحقنة كل اسبوعين ويستمر على ذلك ١٠/٤/٢٠٢١ ، زي الفل وسوف نقلل الابيكسيدون الى ٢ مل، ولكن تشتكي الاخت انه افكار زيادة ومتربص بشخص قريبه وضربه وعايز يضربه بسلاح؟؟ ٦/٢/٢٠٢٢ ماشي على امبول فلونيكسول شهريا مع ابيكسدون ٢ مل وماشي الحال ٢٧/١٢/٢٠٢٢ مازال متحسن جدا لدرجة انه بيقلل الفترة الزمنية للحقنة وبيخدها كل ٢٥ او نص كل ١٢ يوم، وحتى حاليا يعيش وحده لان الام اصيبت بجلطتين وتعيش في رعاية الاخوة البنات، وعايز يبطل سجاير ٢٠/٢/٢٠٢٣ ملخبط في الحقنة ومؤخرا بقى يخدها كل عشر ايام، ولو قلل يحصل اعراض ومع هذا اصبح بطيء وكسول ، ٣٠/٥/٢٠٢٣ ١٥/١٠/٢٠٢٤ مش لاقي الحقن طويلة المفعول، وناشئ على ابيكسيدون ٣ مل ومتمسك بالحقنة وشكله مستقر لكن متمسك اوي بالحقن ولكن حصله جلطة في العين، وماشي على دوا سيولة، ٢٢/٤/٢٥ ياخذ الحقنة كل اسبوع والغريب انها لا تكفيه بعد ذلك
