Social	- الاسم: يوسف كريم يوسف - الترتيب بين الأخوة: الثالث من ثلاثة - تاريخ الميلاد: 05/05/2014 - طالب فى الصف: الأول من المرحلة التمهيدية - السن: 4 سنوات و8 شهور - جهة التحويل: الأهل - تاريخ التطبيق :29 /02/2019 تقرير نفسى • سبب التحويل: تم تحويل يوسف من قبل مدرسة البشاير بسبب شكوى المدرسة من أنه مفرط الحركة، كثير الكلام مما يؤثر سلبًا على زملائه، مندفع الأداء يعانى من انخفاض قدرة التركيز اثناء الشرح، يرفض الترديد مع المدرسة بالإضافة إلى وجود العديد من السلوكيات السلبية المتمثلة فى عدم اتباع التعليمات الموجهة اليه، اللعب بعنف مع الزملاء، والقيام بحركات تثير ضيقهم مثل الامساك بالشعر والأذن. كما أضافت الأم انه حركى فى المنزل خاصة أثناء الطعام، يميل للأمساك بالأشياء فى يده.، يرفض إداء الواجبات • معلومات دالة و منتقاةمن التاريخ السابق للطفل تم أخذه من الأهل: - الولادة : كانت قيصرية . - أظهر تأخر فى الكلام حيث بدء فى نطق جملة من كلمتين عمر عامين و7 شهور ولازال يخلط بين الحروف. - لا يوجد تأخر فى سائر مظاهر النمو الآخرى. - يعانى من عادة مص الاصابع وتشير الأم انه كان لدية تلك العادة وهو فى بطن الأم. - تشير الأم أنه يرفض تناول بعض الأطعمة . - بستطيع يوسف الكتابة بكلتا اليدين.. التقييم النفسى والملاحظات الأكلينيكية: كان يوسف أثناء التقييم متعاونًا ، يظهر أداءا ايجابيًا فى الأنشطة الموازية لعمره الزمنى وإن أظهر العديد من مظاهر اضطراب نقص الانتباه وفرط الحركة حيث كان كثير الكلام بشكل مبالغ، متململ الجلوس يعبث بالأدوات المحيطة ، لا ينتبه للتفاصيل . ولقد تم تطبيق مقياس كونرز(Conner's Rating Scale) يقيس هذا الاختبار المظاهر التالية وقد أجابة الأم على بنود الاختبار وكانت النتائج كما يلى: Guidelines T/ Score Subscale Mildly Atypical (possible significant problem) 61 Oppositional (A) Markedly Atypical(Indicates significant problem) 76 Cognitive problem/Inattention(B) Mildly Atypical (possible significant problem) 65 Hyperactivity (C) Markedly Atypical(Indicates significant problem) 72 Anxious-Shy (D) Mildly Atypical (Possible significant problem) 58 Perfectionism(E) Slightly Atypical(Low Scores are good :not a Concern) 42 Social problem (F) Slightly Atypical (Borderline: should raise concern) 56 Psychosomatic(G) Markedly Atypical(Indicates significant problem) 84 Conner's ADHD Index(H) Moderately Atypical (Indicates a significant problem) 83 Conner's Global Index Restless Impulsive(I) Moderately Atypical (Indicates a significant problem) 67 Conner's Global Index: Emotional Labiality(j) Markedly Atypical (Indicates significant problem) 74 Conner's Global Index: Total(k) Markedly Atypical (Indicates significant problem) 82 DSM-IV :Inattentive(L) Mildly Atypical (possible significant problem) 61 DSM-IV: Hyperactivity/ Inattentive(M) Markedly Atypical (Indicates significant problem) 73 DSM-IV: Total(N) تشير النتائج السابقة إلى وجود ارتفاعات دالة فى مختلف المظاهر التى يقيسها المقياس كما هو موضح بالجدول فيما عدا Social problem 2- كونرز صورة المدرسة : Conner's Rating Scale يقيس هذا المقياس المظاهر التالية . الابعاد المدرسين Conner's ADHD Index Hyperactivity Cognitive problem/Inattention Oppositional 1 76 90 80 82 2 79 90 66 73 3 75 90 77 72 تشير النتائج السابقة إلى اتفاق مختلف المدرسين إلى وجود مظاهر دالة لاضطراب نقص الانتباه وفرط الحركة مصاحب لاضطرابات السلوك لدى يوسلاصة: .
