Diagnosis/Notes
Social	- الاسم : إياد إبراهيم مصطفى . - تاريخ الميلاد: 1/7/2014. - السن: 13 سنة. - الترتيب بين الاخوة: الثانى من ثلاثة ( جومانا : 15 سنة ، ماذن : 9 سنوات). - طالب فى الصف : الأول من المرحلة الإعدادية. - الأم ربة منزل - الأب : صاحب شركة متزوج من آخرى منذ 6 سنوات. الشكوى على لسان الأم: تشير الأم أنه ولد ذكى، حساس جدا ، شاطر فى المدرسة مفيش شكوى منه خالص ، إلا أنه بدأ يعانى من عام تقريبًا من تلعثم فى الكلام وقد أخذت خلال هذه الفترة أشكال مختلفة حيث كان فى البداية يكرر الحرف الأول وبعدين تطور بأنه مع كلمة معينة يخبط قدمة فى الأرض، وحاليًا بيعمل حركة باليد أمام الفم أثناء الحديث . - تذكر الأم أنه فى عمر 3 سنوات و6 شهور تقريبًا كان يعانى من تلعثم فى الكلام وقد تم عرضة على طبيب متخصص فى التخاطب ولم يبدى أى سبب له. - تشير الأم أنه يوجد خلافات بين الزوجين منذ 6 سنوات وقد كانت تصل إلى التعدى اللفظى والبدنى على الأم وكان يحدث ذلك أما الأبناء. - تشير الأم أنه حتى الأن لم يتحدث (معها أو مع الأب على زواج الأب من آخرى ولا حتى يسئل عن عدم تواجد الأب فى المنزل فى أوقات كثيرة) على الرغم من معرفة الاخت الكبرى للموضوع . - هو كمان مش بيعرف يواجه الأب فى أى حاجة عايزها أو مش عايزها، عايز دايما يكون مثالى امامة. وتربطة بالاب علاقة قوية مش بيحب يزعل الاب خالص. - بيعانى من مخاوف من البقاء بمفردة ومن عادة قضم الاظافر. - الخالة: تعانى من وسواس قهري واكتئاب وتعالج على مستوى العيادة . التقييم النفسى : اياد وسيم وبنيانه الجسدى متوافق مع عمره الزمنى، كان فى بداية التقييم يبدى دهشة من موجوده بالعيادة يظهر تلعثم بالكلام مصاحب بحركة باليد اتجاه الفم بشكل متكرر ، وعند الحديث معه فى البداية أشار فى لوجود خلافات بينه وبين الأخوات مبررا ذلك أنه بيخاف عليهم ، بس هما عايزين يعملوا اللى فى راسهم ( مثلا مش بيحب اخوه يعد امام التلفاز مدة طويلة )، عبر كذلك عن وجود أفكار كثيرة فى رأسة خاصة عندما يكون غير مشغول بأى نشاط وخاصة فى المنزل أو اعد لوحده ( تدور الافكار حول ايه اللى هيحصل لو ماما أو بابا حصلهم حاجة ، انا هتصرف ازاى) . بينتج عنها مظاهر توتر وقلق وبيظهر على جسمة انه بيبقى مشدود ، بس الفكرة بتروح لم - أشار أنه لو ماشى فى منطقة مش عارفها بيبقى مركز فى وشوش الناس ( عشان يحمى نفسه منهم). - كما عبر عن ضيقة من عدم الالتزام معه فى المواعيد خاصة الاصحاب بس لو اعتذر له عادى الامور بتعدى ومش بيضايق. - ملحوظة :لم يشير إلى مشكلة التلعثم التى يعانى منها (وعند سؤاله عن تلك المشكلة أشار انه بيضايق منها ولكنه بدء يتعامل معها خلال الفترة السابقة) بأنه ياخد نفس قبل الكلام.، أثناء الحديث كان هناك دموع فى عينة. - حضر اياد مرتين فقط خلال الاسبوعين السابقين . - ملحوظة تم الاستفسار منه عن اعراض ocd / Depression / Phobia ولم يظهر دلالة. محتاجة توجيه الاسئلة للام وتطبيق بعض الاختبارات (cbcl). ملحوظة اجاب اياد على اختبار قلق الاطفال وحصل على 16 درجة وهى ما توقعة فى الفئة البسيطة من حيث القلق. الخطة العلاجية: - علاج دوائى للازمات العصابية .(ملحوظة الجلسة الثانية أشارت الام أنها لحظة خلال الاسبوع الماضى انخفاض حدة اللازمات وكذلك مظاهر التلعثم ... وهو ما تم ملاحظته خلال الجلسة الثانية. - تم التدريب خلال الجلسة الاولى على الاسترخاء التنفسى والتوصية بالمواظبة عليه ثلاث مرات يوميًا. - جلسات علاج معرفى سلوكى لتدريبة على اكتشاف الافكارالسلبية المسببه للمظاهر القلق والتعامل معها بدأت بالفعل ، مع التدريب على توكيد الذات، - اشراكة فى جروب المراهقين الذى يتم تحضيره بعد رمضان. - جلسة مع الاهل للتدريب على كيفية التواصل مع اياد لم تتم بعد. - لو لم يظهر تقدم خلال الشهرين القادمين سوف يتم عرضة على اخصائى تخاطب.
