والاب متوفي وكان لواء شرطة والام عميدة جيش وتوفت من ثلاث عشر سنة، وهي كانت شديدة وعاملة البيت معسكر وعقوبات ونظام صارم، ، وهي خريجة السن المانى تعمل بإحدى الشركات السياحية مطلقة ولديها بنت وحيدة 9 سنوات، تشكو من وجود حالة هستيرية خلالها يظهر تهتهة فى الكلام وفقدان للذاكرة بشكل مؤقت ونسيان للكلمات وتعرق شديد ورعشة، وضربات قلب سريعة ونشفان ريق وأحيانا حالة شبه إغماء تذكر المريضة انها تزوجت الزوج الأول ثم طلقت بعد ٣ سنوات من الزواج وكانت حامل رغم ان هذا الزواج كان مرفوضا من الأسرة وقاموا بمقاطعتها، ثم تزوجت بشخص اخر كزوجة ثانية رغم رفض الاهل ويبدو الزوجين كانو يأذونها الا انها لازلت تحب الشخص الزوج الثانى ويرفض الاهل تلك العلاقة وهناك ضغوط حالية من الاهل للزواج من احد الأشخاص القريبين من الأسرة وهى رافضة قد بدأت الحالة منذ ١٣ عاما عقب وفاة الام التي توفت في المستشفى وهي جوارها وكان عمرها ١٥ سنة وفقدت وعيها ثلاث ايام مع شلل في الساقين وكانت على كرسي عجل وفقدان الاحساس بالنصف الاسفل لمدة ست شهور وفجاة خفت، ، حيث تذكر انها دخلت فى حالة اكتئاب وذهبت إلى د غادة الخولى وتحسنت الحالة بعد ٦ شهور، ، ثم تزوجت الزوج الزوج الثاني كزوجة تانية وتركت شغلها بناء على اعتراضه، وذهبت للعيش معه في السعودية تاركة البنت مع الزوج الاول وبعد ثلاث سنوات فجاة طلقها بسبب مشاكل الزوجة الاولى، وعانت من تنميل في جسمها وغمامة على عينيها واستمرت نوبات انشقاقية مفاجاة في شكل سرحان شديد انا مين وفين ومين دول، وحرارتي تطلع ٤٠، وصعوبة في التعامل مع الناس وبالفحص كانت الاعراض الحالية سرعة انفعال، خوف في الاماكن العامة،خمول،بكاء ونقص الشهية،لوم النفس وتقلب مزاجي وحدة وانقباض، تسارع القلب ،غثيان،ضعف جسدي وحار بارد وتبدد الواقع والشخصية، ، ، وسوف تاخذ بروزاك ٢٠ Diagnosis/Notes Social Allergy Family History Family History No Yes Follow Up مسار 12 جلسة 24/3/2024 محمود سعد تم تدريب المريضة على التعامل مع الأعراض الهستيرية والانشقاقية كما تم تدريبها على التعامل مع الأعراض الاكتيابية وبالفعل التزمت بما تم تعلمه واظهرت تحسن كبير فى الأعراض الاكتئابية حوالى 70% وتحسن فى الأعراض الهستيرية 60% على الأقل ولكن مازلت تعانى من الاعراض الانشقاقية بقوة ودون تحسن حيث تقوم بمحاولات للانتحار أثناء النوم وهو ما أكده الاخ، رغم أننا عملنا عليها من خلال فك الصراعات المتعلقة بوفاة الام ووفاة الابن، وربما توجد صراعات أخرى مرتبطة بطليقها تعذر العمل عليها بسبب أنه كان مريض سابق عندى ٦/٤/٢٠٢٤ مازالت تمشي وهي نائمة وتقف فوق سور البلكونة ومرة خدشت ايدها بالسكينة، والاخ بدا يخاف جدا عليها وممكن نزود البروزاك الى ٤٠ مع ابيكسدون واحد مل ، مسار 7 جلسات اضافية 24/6/2023 محمود سعد. تم تدريبها المريضة على التعامل مع الأعراض الهستيرية والإنسانية والاكتئابية، وقد تحسنت بشكل افضل على تلك الأعراض وتم اكتشاف ان ابنها عايش وان الأخ سجل الولد باسمه مما أدى إلى بعض المراجعات فى الحالة وأعتقد انها قد تنهار فى اى وقت من الأوقات، كما انها ترفض رؤية الولد ٩/٦/٢٠٢٣ ماشية على اتنين بروزاك ٤٠ مل وواحد مل ابيكسيدون والامور مستقرة وبخصوص الولد المحامي قالها تطلع على المانيا بالولد وهناك وتكتبه باسمها ثم تعود مصر وتبلغ رامي به وكانها
