واخت، الاب مدرس والام مدرسة، وهو هندسة ويعمل مهندس، وغير مرتبط (فاتح خطوبة)، ويعيش مع الاسرة في الشرقية، وقد بدأت الحالة في ديسمبر 2023 عندما انتقل لشغل جديد، واخد ورديات مسؤول عنها وبدأ يتوتر جدا ً مع بطء في التفكير وصعوبة في الفهم والتذكر والتركيز، ثم ضرب المشرف عليه وبدأ مع دجال ثم ذهب الي طبيب نفسي واخد مودابكس لمدة قصيرة ثم اولابكس وابيكسيدون وادوية منشطة للمخ، ويبدو مع زيادة الضغط عليه تعرض لنوبة ذهانية خفيفة بدأ بالعنف قم مع الدجال، وحاليا ً هو اهدي كثيرا ً ولكن مازال يعاني من الاكتئاب وحاليا ً علي مودابكس، وهو عموما ً اجتماعي وجو البيت متعاون ولكن علاقته بخطيبته كانت جيدة ولكن سابها مع المرض، وبالفحص مع المرض كان يعاني من صداع، سرعة انفعال، اعياء، نقص الرغبة، حساسية، لوم، نسيان، سرعة الاثارة، عدم الثقة، الشك في الناس، بكاء، نقص الشهية، فزع، قلق، خوف، تسارع القلب، ضلالات المراقبة وبعض الهلاوس، توتر، ارق، مجادلة، التململ، الذنب، والاعراض الحالية النسيان، التركيز، بطء وفقر التفكير، تململ وبالتالي هو شخص لا يعاني من اضطراب شخصية ولكن تعرض لأزمة كبيرة في العمل ودخل في اكتئاب ثم والتركيز والذاكرة والادارك وانه يشعر بقصور فيهم شديد بعد تجربة الذهان التي مر يها وانه يشعر مجرد احساس انه عندما يشاهد شي او يقرا مقال لا يتذكر بسهولة كل التفاصيل وسرعان ما يقارن نفسه بوضع مثالي كان يريد أن يكون فيه ، د.نبيل شافة الاسبوع الماضي وكان شايف انه ممكن ينزل شغل ويتحرك وانه افضل مجرد ماجاء الجلسة قالي انا هشتغل ازاي واما حاسس اني قدراتي العقلية لسة متدهوره في حين ان ملاحظ عليه في البيت من اهله انه كويس ومركز ومنتبه وايضا في الجلسات بيكون منتبه ومركز كويس .. هي الفكرة المسيطرة عليه بشكل ملح اقرب للوسواس انه بعد نوبة الذهان التي تعرض لها تدهورت قدراته العقلية والادراكية) الانتباه والتذكر والادراك والتفكير وربط الافكار ببعض وانها لن تعود وانه لن يستطيع العمل او السفر بسبب ذلك وتظل الفكرة مسيطرة عليه لدرجة تعيقة عن اي حركة خارج البيت لخوفة من الفشل وتجعله احيانا يفكر في الانتحار ويقول انه بيطرد الفكرة من دماغة وتم استشارة د.نبيل والاتفاق علي تحويلة ل د.محمود سعد او د ابراهيم يونس للشغل معه cbt مسار 7 جلسات 21/4/2025 محمود سعد تم تدريب المريض على التعامل مع الافكار الوسواسية خصوصا المتعلقة بضعف الوظائف المعرفية ورغم مقاومه الشديدة الا انه اخبر بتحسن 60% فى التعامل مع الافكار، ولكن رغم ذلك كثير الشكاوى، وعدم الرضا، ويبدو انها سيحتاج للمزيد من الجلسات الاولى انها انقطع منذ شهر رمضان وحتى الآن عن الجلسات ٢٢/٤/٢٠٢٥ مصر انه لا يتحسن والسبب في الذهان رغم تحسنه مع محمود سعد وقد اقرت مرة اخرى انه وسواس قهري وهو حاليا على اريبيبرازول ١٥ ومودابكس ١٠٠ وسوف اقلل جرعة الاريبيرازول
