والأم ، الأب شيف والأم ست بيت ، ومعها اداب عربي ودبلوم تربوى وتعمل مدرس عربي ابتدائي ، لم يسبق الزواج ، وتعيش مع الأب والأم وقد بدأت الحاله منذ 5 سنوات بافكار وسواسية ملحه متنوعه حول اللبس والحجاب والصحه والمرض وخلافه ورغم طول المرض وتأثيره عليها لم تذهب الى طبيب نفسي ، وعانت من اعراض جسديه بسبب تلك الأفكار وبالفحص هى بنت عاديه لأب طيب وفي حاله وام عصبيه وشديده وتعاني من وسواس القهرى (افكار غريبه عايزة تعرض نفسها عليا ومش متقبلاها) ، مع بكاء وفزع وخوف وانقباض وخمول وقلق والدونيه والتوتر ، التململ ،(الأب غير مقتنع بالعلاج النفسي والطب النفسي) وملح ومزعج – وسوف تأخذ فارفارين وتكمل مع شيماء ٨/١١/٢٠٢٣ جاءت بعد شهر وتشتكي من عدم التحسن نهائيا وسوف تاخذ ابيكسدون نص صباحا ومساء Diagnosis/Notes Social Allergy Family History Family History No Yes Follow Up ١٤/٢/٢٠٢٤ لم تتحسن مطلقا بالفافارين وسوف تحول الى فلوزاك وتكمل مع محمود سعد او ابراهيم للشغل على الوسواس مسار 6 جلسات 14/5/2024 محمود سعد تذكر فى الجلسة الأولى بتجيلى افكار بأحلام يقظة عبارات عن ناس بتقنعنى انى مصابة بالفصام والذهان، ولو شايفه كورسى تجيلى فكرة انتى بيتهيقلك، ولو حد بيتكلم يخطر فى بالى مش سمعاه وده بيخلينى اسأل ولو حاجة فهماها الاقى فكرة بتقولى العكس، وده بيخلينى ادخل فى حوار كبير زى ما يكون فيه اشخاص واحنا بنجادل معاهم، وتجيلى افكار ان الحجاب بيبين جمال المرأة،والحجاب ربنا فرضه عشان يثير الشهوات لكن العرى مش بيلفت النظر، واتخيل أشخاص كأنى برد عليهم، والتعب مش بتأثر على الجمال، وساعات تقولى جميل معناها قبيح، ولو عندى ألم فى المعدة تجيلى افكار تقولى انتى معندكيش ألم فى المعدة، لان الألم حاجة مش بنحسه، احنا بنحس بالراحة، ولما أقف فى المراية تقولى دى مش صورتك، وتقولى ملامحك فيها شبه من القرد. تم عمل 6 جلسات بشكل متقطع نتيجة لعدم انتظام المريضة، رغم عمل تخفيض كبير لها لتحسين انتظامها دون جدوي، تم تدريبها على التعامل مع أفكارها الوسواسية، وتخبر بتحسن الحالة بنسبة 50% ولكنها ملخبطة فى وصف التحسن وفى كيفية أداء التدريبات، كثيرا ما تقوم بعمل التدريبات بطريقة خاطئة رغم تدريبها مرات عديدة عليها، واظن ان نسبة التحسن مرضية بالنسبة لعدد الجلسات وطبيعة المريضة ١٥/٤/٢٠٢٤ الوضع كما هو تقريبا وتم انقاص البروزاك الى كبسولة واضافة انافرانيل ١٥٠ وابيكسيدون نص مل ١٥/٧ تقول لا يوجد تحسن اطلاقاً ودماغها شغالة جامد جدا، ونفس الصراعات الوسواسية، وشكلها متدهور وتشتكي كثيرا من الموضوع، وسوف نزيد الانافرانيل الى ٢٢٥ مع
