CC: Obsessive thoughts HPI: Condition started at age 11 with obsessive thoughts about dirty hands and compulsive hand washing and avoiding touching anything. The obsessions varied every year or 2 years with obsessive checking on locking doors, prayers وضوء, thinking he would hit somebody while driving so he would video tape his drive and review it after finishing the drive. Lately, over the past year, the obsession thoughts are coming in the form of writing swear words on his answer sheets in exams with compulsive checking and taking a picture with his phone of the answer sheet and reviewing it multiple times. Also obsessive thought of دعاء against his parents whenever he hears prayer calls with compulsive behavior of videoing himself during prayer calls and then reviewing the video to check if did or not. 2 months ago he also had intrusive thoughts that questioned his sexuality “فكرة بتقولي اني شاذ” Recently he developed mood symptoms with anhedonia, depressed mood, weight gain, death wishes (no Suicidal ideations) He first sought medical advice 1 year ago and was prescribed Anafronil 75mg nightly and Faverin 100mg AM. He was no compliant with his medications. FH: none PMH: Volleyball injury at age 16 resulting in right ACL and meniscus tear with surgery and rehabilitation during last year of ثانوية, with full function now.
