She describes him as kind, occasionally aggressive, irritable she has good relationship with him (From mother he smokes weed 5x daily at least and drinks a bottle of wine daily) Mother is 50yo, artist and worked various jobs to support the family, (she is my patient). Habiba describes the relationship as irritable for the past year since "I changed and started noting things in her that are similar to me and started blaming her for them" Past Psych: in 2014 counseling after a breakup for 1 year on & off. No PMH FH: mother depression Complaint: "Mother asked me to be here. I am ok." Schema: paternal critic dominating the conversation throughout. childhood schema of trust vs mistrust due to unstable home environment leading to adaptive behavior of unrelenting standards. Personality traits: Black and white thinking. Feeling of inadequacy No depressive sx, normal mood and reactive affect. No SI, No HI. No psychosis. No obsessions. Patient was guarded with the interview due to me being her mother's psychiatrist Plan Supportive psychotherapy with Ms. Haya with focus on errors of thinking and DBT skills for emotional regulation (she says she gets angry easily with her mother) Follow up in 6 weeks Diagnosis/Notes Social Allergy Family History Family History No Yes Follow Up Session 1: Discussed that the main complaint for coming to therapy is realizing (2 years ago) that the communication among her and her family is not right (described as passive aggressive, belittling, and exaggerated emotional responses). We set SMART goals for therapy regarding regulating the intensity of her emotional responses which will reflect in less conflicts between specially her and her mother. We worked on identifying the physical and situational anger triggers, relaxation by breathing and PMR, and taking a time out. Habiba: Session 2: She is anxious due to exams, end of work contract, and having to decide on whether to move to another place. Her thinking was rational, but we explored pros and cons for the decision until she reached one. It was affecting her sleep, so we agreed on sleep hygiene/routine tips. We explored her anger triggers and started working on knowing the functions of emotions and challenging thoughts that her emotions are reasons people (may not) love her. Rest of skill will be continued next. Session 3: Mood was fine. She took the decision to move and is comfortable with it. We continued the anger trigger hierarchy. We learned describing emotions and checking the facts for emotional regulation; she will practice it to change her interpretation too. Session 4: her mood was good. We discussed not always being convinced of the new interpretations she thinks of when checking the facts of anger triggering situations and we tested the primary thoughts and agreed on doing this and mentally practicing the new thoughts regularly. We learned and will practice opposite action for emotional regulation.
