Summary	4 / 5 /2016 السيد/ محمد عصام، 18 سنة، تالته ثانوى American، يعيش مع الأم والأخت المطلقة وأبنها بالمهندسين (الأب متوفى من 5 سنوات). شكوى المريض: بعد وفاة بابا بسنة ونص أبتديت اكتئب، أبتديت أنعزل وأخرج لوحدى، الحالة أبتديت تتحسن من شهر وشوية عشان بحاول، بتجيلى خنقة، قلبى ينبض جامد وأحس أنى هموت. شكوى المبلغ: (الأم) راح قبل كدة لأخصائية ولمدة سنة ومافيش تطور، بعد الوفاة أبتدا ينعزل ومايهتمش بلبسة، فى المدرسة طلبوا يروح لأخصائية، السنة اللى فاتت أتنقل لمدرسة تانية، ورفض يروح المدرسة وأبتدا يتخنق جداً. التعقيب: - Anxiety attacks or panic attacks. - Auditory, وفيه أصوات بسمعها من كام شهر, allocution ??. - أحساس بالضعف بعد وفاة الأب منذ 5 سنوات وخصوصا ً مع ظهور مشاكل بالمدرسة أدى إلى الأنعزال، ثم بدأ يفتح مرة آخرى تدريجيا ً ومؤخرا ً بدأ ظهور أصوات غريبة (زن أو زعيق). - Depression with psychotic features. R\ Cipralex 1\2x1- Apexidon 0.5 mg 1x1.

Follow Up	٢/٧/٢٠٢٠ جاء منكوس الشعر وبذقن ويتكلم بصعوبة وبدون تفاصيل وقال انه ماشي كويس وبينجح في جامعته وأموره كويسة ولكن غير اجتماعي وكذلك فيه مشاكل كثيرة مع الاخت المطلقة، وبدا من سنة التوقف عن الخروج وممارسة اَي أنشطة اجتماعية او رياضية مع شكاوى جسدية وذهب لطبيب قلب ولَم يجد شيئا، ومن اربع ايّام بدا يشعر بخنقة وهو عموما غير مبسوط ويبدو ان هذا الرجل مر في تاريخه الشخصي بلخبطة جامدة بالذات اثناء المدرسة عندما اضطر الى اعادة السنة وبعدها اضطر الى ترك المدرسة وبدا بعدها التدهور ويحتاج الى شغل طولي مستمر وسوف ياخذ بروزاك وبوسبار، ٦/٩/٢٠٢٠ احسن كتير واكثر اقبالا ومزاجاً واصبح اكثر اختلاطا واجتماعيا والتوتر اقل وبيتواصل مع أصحابه، كما انه يتواصل مع أخصائية نفسية تبعه ولكن بشكل غير منتظم ٦/١٢/٢٠٢٠ ماشي على بروزاك صباحا وبوسبار ٢٠ وكويس واجتماعي والتفكير اكثر استقرارا والدراسة جيدة ومازال مستمر مع الدكتورة المعالجة ، وسوف نزيد البوسبار الى ٤٥مل لانه ويشتكي من الأرق ١٠/٣/٢٠٢١ كويس او كما هو واجتماعي وظريف وبيذاكر ولكن يشتكي من كوابيس واحيانا صداع وسوف نجعل البوسبار صباحا ومساء ٣٠/١٠/٢٠٢١ ماشي على بروزاك وبوسبار ٣٠ وزي الفل ولديه وعي جيد بذاته وقدراته واحتياجاته وهذه احسن مرة شفته فيها، ولكن المشكلة في الصداع ٨/١١/٢٠٢٣ جاء بعد سنتين ويشتكي فقط من التوتر ويرجع ذلك الى انشغاله بالمشروع ولاحظت عليه تاخر في الرد كما انه وحيد وفقير في الحياة الاجتماعية وسوف يبدا حضور جروب
